New Patient/Client Facesheet
A custom report that provides a quick snapshot of patient demographics and medical history upon registration into a member organization program.
Quality Measure Dashboard
Customizable for your organization’s programs, population, and quality measures of interest.
Dashboard Features
The dashboard displays can be manipulated and filtered to display claims or clinical data, sites and insurance plans, demographic features, and patient cohorts designated by disease or other conditions. Different display options include a time series, a bar chart snapshot across sites (with bar widths proportional to population size), a geospatial view, and a “speedometer” display showing progress against the measure target. The displays are dynamic and change as filters are selected or unselected.
Once generated, measure displays can be downloaded as static images or as line-level patient details for the current selection.
