Requesting a Report of Who Has Accessed Your Data

As a patient, you have the right to find out who has accessed your health information through the Bronx RHIO.

Upon request and after proper verification of identity, the patient will be provided with an audit report which contains the following:

  • The name and role (e.g., physician) of each Authorized User who accessed a patient’s Protected Health Information in the prior 6-year period;

  • The Participant through which such Authorized User accessed such Protected Health Information;

  • The time and date of such access; and

  • The type of Protected Health Information or record that was accessed (e.g., clinical data, laboratory data, etc.).

An audit report will be provided to patients at no cost once in every 12-month period.

To request a copy of your report, please complete the Patient Identification Verification form and contact the Bronx RHIO at 718-696-0450 or information@bronxrhio.org to arrange for identity verification. This will allow us to be sure that we’re giving your data to only you.
